The development of healthcare facilities in Indonesia has significantly increased throughout the past decade, consequently increasing solid medical waste generated from various healthcare activities. This waste poses potential risks to human health and the environment due to its infectious, toxic, and hazardous characteristics. Ineffective management of medical waste can result in disease transmission, environmental pollution, and adverse impacts on public health. Law No. 32 of 2009 on Environmental Protection and Management mandates that healthcare facilities manage their waste responsibly through a systematic approach covering waste segregation, collection, transportation, storage, and disposal. However, challenges in implementing this regulation still exist, including limited infrastructure, human resources, and operational procedures in healthcare facilities. Effective solid medical waste management requires a comprehensive and integrated approach involving multiple stakeholders, from healthcare facility managers to government agencies. Technological innovation and effective monitoring systems, as well as continuous training to improve the capacity of human resources, are critical in optimizing waste management. The research seeks to investigate the primary determinants that influence the implementation of Law No. 32 of 2009 in healthcare facilities and to provide recommendations for optimizing the waste management system to comply with the law, ensure effective implementation, and support long-term sustainability.
