NSAID-INDUCED GASTROPATHY DENGAN MANIFESTASI PERDARAHAN SALURAN CERNA ATAS – LAPORAN KASUS

Abstract

Non-steroidal anti-inflammatory drugs (NSAIDs) are widely used for various indications, mostly in cardiovascular and rheumatological diseases due to its antipyretic, analgesic, anti-inflammatory, anti-rheumatic, and anticoagulant properties. Overuse of NSAIDs is a health problem that increases the risk of NSAIDrelated gastropathy. This condition manifests with erosive and/or ulcerative lesions of the gastrointestinal tract. This can then cause gastrointestinal bleeding and subsequent complications such as anemia and shock. Several risk factors were found to be associated with NSAID-related gastrointestinal bleeding, namely age (>60 years), high-dose NSAID treatment, history of gastric ulcers with or without complications, Helicobacter pylori infection. We report a case of gastrointestinal bleeding associated with NSAID use in a 61-year-old woman who self-prescribed NSAIDs for her rheumatological condition. This case report aims to demonstrate the importance of adequate patient education regarding NSAID use and its management. Upper gastrointestinal bleeding in this patient manifested hematemesis and coffee ground emesis. The patient was managed in accordance to the guideline for the management of upper gastrointestinal bleeding as well as a planned assessment and treatment of Helicobacter pylori to lower the likelihood of further bleeding occurrences.
