Urinary tract infection (UTI) is the most common bacterial infection in children. The most common bacterial cause of UTI is Escherichia coli (E. coli). UTI is also common in children with nephrolithiasis. This case report describes a 9-year-old girl with UTI with left nephrolithiasis and bilateral hydronephrosis. Case Presentation : A 9-year-old girl came to the emergency room with complaints of fever since 7 days before admitted. Based on anamnesis, physical examination and supporting examination, the patient was diagnosed with UTI with left nephrolithiasis and bilateral hydronephrosis. Discussion : Diagnosis of UTI in children is still a challenge for health workers. Recurrent UTI is an important risk factor that has the potential to cause permanent kidney damage in children with nephrolithiasis. Antibiotics are effective for most pediatric UTIs. The initial antibiotic selection is based on local sensitivity patterns and the risk of resistant bacteria. The antibiotic management of this case is Ceftriaxone 2x1250 mg accompanied by supportive drugs such as Ondansetron 3x3 mg, Ranitidine 2x30 mg, Paracetamol supp 250 mg. Conclusion : UTI is the most common bacterial infection in children, so early diagnosis and appropriate treatment play an important role in the prognosis of UTI patients.
